What a Health Maintenance Organization (HMO)?  Definition below:

A Health Maintenance Organization (HMO) is a type of health insurance in which a primary care physician (PCP) is responsible for coordinating care.

Within an HMO, the PCP must give a referral for members who wish to see a specialist. The Health Maintenance Organization is based off of the idea of healthcare prevention. Providers that are out-of-network are typically not covered under an HMO plan.
